Position Overview:
Duties & Responsibilities:
- Conduct comprehensive assessments of individuals in crisis to evaluate the severity of their situation and immediate needs.
- Provide short-term intervention strategies, including emotional support, de-escalation, and safety planning.
- Develop individualized crisis intervention plans, identifying immediate action steps and coordinating the appropriate resources.
- Collaborate with crisis intervention teams, law enforcement, or medical professionals during high-risk or high-intensity situations.
- Assist clients in understanding their situation and available options for treatment or stabilization.
- Maintain accurate, up-to-date case files and documentation, ensuring compliance with confidentiality and legal requirements.
- Document client interactions, crisis interventions, outcomes, and referrals in accordance with organizational policies.
- Connect clients to appropriate community resources, including mental health services, housing, substance abuse programs, healthcare, and legal services.
- Follow up with clients to ensure that they are receiving the necessary services and make adjustments to intervention plans as needed.

Requirements
- Bachelor’s degree in social work, Psychology, Counseling, or a related field is typically required.
- At least 2 years of experience in crisis intervention, case management, or social services
- Current Texas Driver's License (upon hire)
- Proficient in MS Office Suite (Word, Excel, PowerPoint, Outlook)
- Excellent communication skills (verbal & written)
- Ability to establish and maintain effective working relationships and foster teamwork in a diverse environment
- Ability to maintain confidentiality when dealing with sensitive information
- Ability to work well with public and staff members
- Must use your personal mobile device for our Multifactor Authentication process
- Must possess the ability to effectively present information in one-on-one and small group situations to other employees and/or managers
- Must be knowledgeable of company protocols, operational procedures and safety policies
- Ability to de-escalate complex situations by using Trauma Informed Care and harm reduction practices
- Ability to coordinate activities closely with others in stressful situations

General Information
- This is a full-time position.
- Days and hours of work are typically Monday through Friday, from 8am to 5 pm.
- This position is required to be available for on-call duties as needed, ensuring continuous support and coverage outside of regular business hours.
- Other days and hours as necessary.
Work Environment:
- This position involves both a professional office environment and fieldwork, requiring travel to external locations for site assessments, meetings, and hands-on tasks.
Physical Demands:
- Must be able to lift up to 50lbs, have the ability to stand, bend, and or sit for extended periods of time.

- Harris County Public Health: Community Health & Violence Prevention Division 1111 Fannin St. Houston, Texas. 77002.
Employment may be contingent on passing a drug screen and meeting other standards.
