Position Overview:
The mission of the Harris County Holistic Assistance Response Team (HART) Program is to reduce unnecessary law enforcement responses to non-emergency 911 calls. The HART program is currently administered by a contract vendor and consists of teams of behavioral health and social service workers and emergency medical personnel who respond to 911 calls requiring face-to-face interventions, low-acuity pre-hospital care, and other social welfare concerns.
Under the Direction of the Holistic Alternative Response Team (HART) Program Manager, the HART Program Coordinator is responsible for the day-to-day administrative and programmatic coordination of the HART contract vendor and collaboration with the Harris County Sheriff's Office (HCSO). The HART Program Coordinator will work closely with internal partners, the vendor, and HCSO to ensure contract, program requirements, and outcomes of the HART Program are met and accurately documented and reported.

Duties & Responsibilities:

  • Coordinates with staff in the Community Health and Violence Prevention Services Division (CHVPSD), internal and external partners, the Harris County Sheriff's Office, and contract vendor to create, design and implement the HART program.
  • Serves as a liaison between the Harris County Sheriff’s Office HART program staff responsible for coordinating and managing the 911 call system to ensure appropriate calls are routed to the HART vendor.
  • Assist HART vendor with processing appropriate administrative documents with HCPH and other County departments such as vendor invoices, reports, and other administrative requirements as appropriate.
  • Conduct regular field assessments to evaluate vendor performance and offer guidance on corrective measures when necessary.
  • Collaborate with the Office of Communications, Education, and Engagement (OCEE) to plan, develop, and implement outreach strategies internally and externally to increase awareness, use, and confidence in the HART program across Harris County.
  • Provide guidance to the HART program assistant on the administrative tasks associated with the HART program.
  • Generate, modify, and organize reports, data and documentation as needed, encompassing daily activity summaries, field observation reports, as well as weekly and monthly HART reports.
  • Monitor, facilitate, and oversee HART referrals to ACCESS Harris by maintaining open communication with a designated ACCESS program staff member(s) to guarantee a seamless transition between the two programs and ensure that HART program participants are successfully enrolled in the ACCESS Harris County Initiative.
  • Other duties as assigned, including responding to an emergency event.

Requirements

Education:

  • Bachelor's Degree in Social Work, Sociology, Public Health, or other related field.

Experience:

  • At least two (2) years' experience in public health, social services, homeless and/or behavioral health operations, and program management.
  • At least one (1) year of experience in Case Management.

Knowledge, Skills & Abilities:

  • Proficient in MS Office Suite (Word, Excel, PowerPoint & Outlook)
  • Ability to prioritize tasks and manages time to ensure that deadlines are met.
  • Knowledge of contract management and oversight principles.
  • Experience developing collaborative relationships with community stakeholders and partners.
  • Experience implementing equity principles and strategies in social service, health, homeless, and/or other social safety net programming.
  • Experience and knowledge of project management tool(s) and methodology.
  • Ability to work cross-departmentally, work within multidisciplinary teams, oversee projects to completion, and measure results.

  • Monday through Friday: 08:00 am to 05:00 pm (flexible)
  • Evenings and weekends as necessary; hours will vary earlier or later based upon division and department events and program needs.

Work Environment:

  • Work environment is typically split between an office environment and field (outdoors).

Physical Demands:

  • Persons in these positions frequently lift, carry, push, and pull objects weighing up to 10 lbs. They also occasionally move objects weighing up to 20 lbs., and sometimes, but rarely, up to 50 lbs.
