Advanced Practice Provider (APRN or PA) Opportunity in the Goals of Care and End-of-Life Planning Department

We are seeking a qualified Advanced Practice Provider with a minimum of five years of experience in advanced practice. Experience in goals of care is preferred.

Schedule: Monday to Friday, 8:00 AM - 5:00 PM.

SUMMARY

The primary role of the Advanced Practice Provider (APRN or PA) specializing in Goals of Care and End-of-Life is to deliver comprehensive therapeutic patient care is to identify patients nearing the end of life, and coordinate/facilitate high quality goals of care conversations with the patient, family, inpatient, and outpatient care teams. The APRN/PA/APP will work with the primary outpatient cancer providers and the inpatient care teams to reach consensus on prognosis and communicate this to patients and families.

Utilizing advanced communication skills, the GOC APP will help patients and families make critical decisions that align with their preferences while helping them understand what is prognostically feasible. These decisions will include but not be limited to advance directives, preferences for CPR and intubation, artificial nutrition, preferences for where to spend the remainder of life. This entails executing evidence-based patient care practices, utilizing resources judiciously, and collaborating closely with the inter-professional healthcare team to plan, educate, and coordinate services effectively. Additionally, the GOC APP plays a pivotal role in providing education and support to patients and their families, with a focus on integrating the patients' expressed values and preferences to enhance comfort and quality of life. Moreover, the GOC APP actively participates in data collection and ensures accurate documentation in the medical record.

This role serves as a cornerstone in advancing the institutional strategic plan's Goal Concordant Care Initiative. With its institutional scope, the GOC APP collaborates extensively with patient care, operations, and clinical leadership across the organization to drive forward initiatives that prioritize goal-aligned patient care and optimize outcomes.

JOB FUNCTIONS

Essential Functions

Provides assessment, triage, and therapeutic management of patient/family needs throughout the disease process as an integral part of an interdisciplinary team.

Acts under the authority and supervision of the attending physician and in accordance with regulations, hospital policy, protocols, and clinical pathways.

Obtains medical histories, performs physical examinations, reviews/orders diagnostic tests and radiographic results, formulates and manages patient treatment plans and outcomes.

Performs as first or second assist on operative cases, using fine motor skills and manual dexterity.

Performs procedures, prescribes treatment, and signs prescriptions in accordance with institution and state requirements.

Screens physician calls to educate patient/family on care and resources.

Have Goals of Care conversations with patients and document.

Makes rounds with interdisciplinary team.

Collaborates in research projects, formulates protocols and applies research to practice.

EDUCATION:

Required: Master's degree from an accredited Physician Assistant (PA) program by the Accreditation Review Commission on Education for the Physician Assistant (ARC-PA), except for Physician Assistants who graduated from a PA program prior to 2013.

Required: Master's Degree in Nursing or Doctor of Nursing Practice. May be waived for applicants who obtained recognition as an Advanced Practice Registered Nurse from the Texas State Board of Nursing prior to 1/1/2003. Graduate education and related Texas Board of Nursing authorization must be consistent with clinical practice area and patient population.

EXPERIENCE:

Required: None.

Preferred: At least five years of experience as an Advanced Practice Provider (APRN or PA). Goal of care experience is preferred.

LICENSURE/CERTIFICATION:

Required: All of the following:

All of the following: PA certification by the National Commission of Certification of Physician Assistants. Licensed by the Texas State Board of Physician Assistant Examiners.

MAY BE REQUIRED: Eligible to be granted limited prescriptive authority by the State of Texas. Must meet the American Heart Association Basic Life Support (BLS), ACLS (Advanced Cardiac Life Support) or PALS (Pediatric Advanced Life Support) if working in one of following department: Pediatrics - Patient Care (113100).

Required:

All of the following: Written authorization from the Texas Board of Nursing to function as an Advanced Practice Registered Nurse (APRN or ACNP or ACNPC or ACAGNP or AGACNP or ACNPC-AG or AGACNP or AGNP or ACPNP or ANP or AGNP or AGPCNP or FNP or GNP or PNP or PMHNP or WHNP or APNP). Related authorization must be consistent with clinical practice area and patient population. Basic Life Support (BLS). Pediatric Advanced Life Support (PALS) If working in the following departments: Pediatric - Patient Care (113100).

MAY BE REQUIRED: Authorization for limited prescriptive authority by the Texas Board of Nursing.
